Behavioral Health RN Coordinator $93,000 – $97,000/year Full-time, exempt Pottstown, PA We are seeking a highly skilled and experienced Behavioral Health RN Coordinator to join our team at CHDC. As the Behavioral Health RN Coordinator, you will be responsible for coordinating and overseeing the delivery of behavioral health services to patients. You will work closely with the interdisciplinary team to ensure that patients receive the highest quality of care. What you will do: The Behavioral Health RN Coordinator is a highly skilled and experienced member of the Behavioral Health team. The Behavioral Health RN Coordinator is responsible for coordinating and overseeing the delivery of behavioral health services to patients. Works closely with the interdisciplinary team to ensure that patients receive the highest quality of care. This role will be critical in ensuring that patients receive the appropriate level of care and support to achieve their health goals. Duties include but are not limited to: Coordinate and oversee the delivery of behavioral health services to patients, including behavioral health consulting and telepsychiatry. Collaborate with the interdisciplinary team to develop and implement patient treatment plans. Coordinating new patient and follow up appointments with providers Provide ongoing education and support to patients and their families on behavioral health issues. Assist in implementation of organization policies, procedures, workflows, and action plans to drive team performance. Supports CHDC on program initiatives, QI endeavors, and opportunities for increasing revenue. Ensure compliance with all regulatory and accreditation standards, including site visits and audits. Maintain accurate and complete patient records Ensure collaboration and communication among multidisciplinary teams to ensure they are coordinated in working towards maximum patient outcomes Respond to telephone calls from patients with behavioral health related questions, gather pertinent information and utilize critical thinking and decision-making skills. Responsible for assisting in medication management/refills. Assist in completion of annual UDS report for HRSA Audit EHR charts for data validation and resolve issues Other duties as assigned. Requirements: Qualifications, Education, and Knowledge Active unrestricted RN license in Pennsylvania required. Certification in psychiatric nursing preferred. Minimum of 2 years of experience in behavioral health nursing required. Strong knowledge of behavioral health conditions and treatments required. Excellent communication and interpersonal skills required. Ability to work collaboratively in a team environment required. Experience in care coordination or case management preferred.
